Consent Forms

For those attending a Woodlore course, special event or workshop, our Consent Form must be filled in and returned to us prior to the commencement of your course.

The Consent Form is necessary for your safety; it records details of allergies and other medical conditions, in addition to emergency contact details etc. Please fill in the correct form - there are separate forms for Adults and Minors. Thank you.
